On this PTSD Awareness Day, we turn to research that deepens our understanding of dissociation and sheds light on why visual practices hold particular promise within trauma-sensitive yoga. Although these methods often appear simple, softening the gaze, tracking movement, and orienting visually, they directly engage neural systems affected by trauma.

Before exploring these developments, it is worth remembering how recently PTSD entered the clinical lexicon. The diagnosis appeared in 1980 in the Diagnostic and Statistical Manual of Mental Disorders (DSM-III) and later in the International Classification of Diseases (ICD) in 1992. Prior to this, individuals experiencing what we now associate with PTSD—dissociation, freezing, intrusive memories, chronic fear—were frequently misunderstood. Their symptoms were minimised or pathologised, and science lacked the frameworks to interpret their experience.

Recognition of PTSD transformed this landscape. It repositioned these responses as adaptations to overwhelming events. As a result, research expanded, public understanding grew and trauma-informed modalities, including trauma-sensitive yoga, began to take shape.

How Neuroscience Has Informed Trauma-Sensitive Yoga

Since PTSD entered diagnostic frameworks, knowledge of trauma has progressed significantly. Neuroscience now shows that PTSD involves changes in multiple brain regions: heightened amygdala activity, reduced prefrontal engagement and disrupted communication between networks. Many clinicians and yoga therapists use these insights to explain how practices such as grounding, breathwork, interoception and mindful movement support recovery.

However, newer findings suggest that dissociation involves a different kind of disruption. Individuals with high levels of dissociation show abnormally strong connectivity between networks that are usually separate, responsible for sensation, memory, movement and attention. Instead of supporting flexibility, this excessive connectivity creates rigidity. The brain becomes overloaded, struggling to transition between states or organise new information. This rigidity may also affect neuroplasticity, reducing the brain’s responsiveness to change.

In addition, research shows a breakdown in the brain’s “small-world organisation.” In healthy systems, local neural clusters perform specialised functions and connect efficiently with broader networks. Dissociation disrupts this balance. The brain becomes globally over-connected but locally under-organised. Activity is high, yet coherence is low. Experience becomes fragmented, difficult to process and hard to integrate into a stable sense of self.

This explains why someone may appear shut down externally while internally experiencing overload. They are not inactive; they are caught in a state of chaotic, disorganised neural activity.

The Visual System’s Role in Dissociation

One of the clearest behavioural markers of dissociation is the characteristic “staring into space.” Although this may seem like withdrawal, it is now understood as an orienting response: an attempt by the visual system to find coherence. The eyes fixate or scan as the brain searches for a focal point that might reduce internal chaos. Yet the glazed quality of the stare suggests that the attempt is unsuccessful. The system is overwhelmed.

These findings help explain why visual practices have a unique role in trauma-sensitive yoga. Because the visual system is already activated during dissociation, guiding visual attention may support reintegration more precisely than general movement alone.

How Trauma-Sensitive Yoga Can Respond to Dissociation

Once the gaze is responsive and the client can follow cues, eye movement practices may be layered in—left to right, near to far, or smooth visual tracking.

These methods are not used for trauma processing. Instead, they support orientation, coherence and regulation. They help the nervous system interrupt the “glitch” that occurs during dissociation by aligning with what the brain is already attempting to do.

During these practices, breath can be introduced slowly. Breathing shifts neural activity, and when combined with visual focus, may increase the likelihood of moving the system toward regulation. Although we will explore mechanisms in a future piece, it is sufficient here to say that the combination of gaze and breath enhances the potential for change.

Connecting Visual Practices with Broader Trauma Modalities

Visual practices in trauma-sensitive yoga sit alongside a wider landscape of trauma approaches. EMDR and IEMT both use eye movements to influence memory-related processing. Brainspotting focuses specifically on the subcortical orienting response, using visual fixation to access deep, implicit emotional material. Although these techniques operate within different therapeutic scopes, all acknowledge the power of the visual system in trauma recovery.

For yoga therapists, the relevant takeaway is not to perform trauma-processing techniques, but to understand why visual cues matter. When used appropriately, they support regulation in clients experiencing dissociation, helping them re-establish coherence and reconnect with their environment.

Why Trauma-Sensitive Yoga Is Uniquely Placed to Integrate These Insights

The strength of trauma-sensitive yoga lies in its ability to meet the nervous system directly. The practice emphasises interoception, breath, embodiment and presence—all essential components for working with dissociation. These insights confirm what many practitioners have observed for years: when we meet the nervous system where it is, we increase the likelihood of supporting change, even in subtle or non-verbal states.

These reflections are not an invitation for trauma-sensitive yoga teachers or yoga therapists to use eye movements for trauma processing. That work requires specialist training and lies outside the scope of most practice. What is within scope is the ability to notice dissociative markers, such as fixed gaze or scanning, and to gently offer visual practices that support regulation. When used with discernment, these tools can help re-establish coherence and offer clients a way to return to themselves with safety and dignity.
