Breath Holds: The Physiology, Psychology and Transformative Potential of Kumbhak

Yoga has always paid immense homage to the breath, recognising it as a primary gateway to transformation. Breath holds are not merely the movement of air into the body, though it sustains physical life. It is an expression of prana, the vital force that animates experience. The way we breathe, the rhythm, depth and quality, shapes our internal landscape. And just as breath influences perception, intentional control of the breath can begin to reshape it.

A deliberate pause after the inhale, antara kumbhaka, or after the exhale, bahya kumbhaka, has long been viewed as a threshold into refined states of awareness. These moments create heightened clarity, greater internal focus and a loosening of habitual mental activity.

This was not metaphor.
It was knowledge preserved through disciplined practice and confirmed through direct perception. Yogic traditions recognised that interrupting the breath draws the mind inward. Breath is tied to survival, and its suspension interrupts the body’s automatic rhythm, creating an immediate shift in awareness.

Modern science largely supports this. Holding the breath interrupts rumination because neurons require a constant supply of oxygen to sustain activity. When oxygen levels begin to decrease, the brain redirects attention toward maintaining life. Thought narrows. Internal noise quiets. But current research reveals that breath holds, when used deliberately and within safe limits, create more than a temporary state change. They can induce long-term physiological adaptations that support cellular resilience, nervous system balance and improved stress regulation.


The Physiology of Breath Holds

Two internal processes unfold during kumbhak.

Carbon dioxide begins to rise. This does not happen because we inhale it, but because we continue producing it while temporarily pausing the exhale.
At the same time, oxygen levels gradually fall as the body uses what is available without replenishment.

These effects are known as hypercapnia, a rise in carbon dioxide, and hypoxia, a drop in oxygen. Although harmful in extreme or prolonged states, when created gently and intentionally they stimulate powerful adaptive responses.

The Role of Carbon Dioxide

Often dismissed as a simple waste product, carbon dioxide is a crucial signalling molecule. It regulates blood pH, supports cerebral blood flow and influences immune activity.

During a breath hold, a slight rise in carbon dioxide:

• expands blood vessels in the brain, improving circulation
• triggers the release of antioxidant enzymes such as glutathione peroxidase and superoxide dismutase
• modulates inflammation without suppressing necessary immune processes

If carbon dioxide rises too high for too long, the body shifts into respiratory acidosis, a harmful state where the acid–base balance is disrupted. This is not what occurs in safe kumbhak. In well-applied breath holds, the rise in carbon dioxide is brief, controlled and used to build resilience.

One of the most interesting effects is on the chemosensory reflex. These receptors detect rising carbon dioxide and send the signal to breathe. With repeated exposure, the reflex becomes less reactive. The body learns that a small rise in carbon dioxide is not dangerous. This fosters a calmer response to internal stress, which is why breath-based practices often strengthen emotional tolerance.

The Role of Oxygen

As oxygen gradually falls during a breath hold, another response is activated. Mild, short-term hypoxia triggers the expression of hypoxia-inducible factor 1 alpha, which supports cellular survival under reduced oxygen conditions.

Through this mechanism:

• new blood vessels form, improving long-term oxygen delivery
• cells become better at producing energy with less oxygen
• metabolic flexibility increases
• emotional and physical stress tolerance improves

Breath holds have also been studied for their effects on apoptosis, the natural process of cellular death. When overstimulated, apoptosis can remove healthy neurons. Controlled hypoxia appears to support regulation of this process, protecting vulnerable neural networks.


What the Research Shows on Breath Holds

A 2024 review by Tregub and colleagues synthesised research on intermittent hypercapnic hypoxia, the very state induced by kumbhak. The review highlights several measurable effects:

• reinforcement of the blood–brain barrier
• increased activity of antioxidant repair systems
• healthier regulation of apoptosis
• improved cellular energy production

These findings resemble clinical techniques such as ischemic preconditioning, where brief, safe stressors strengthen the body’s resilience. Kumbhak has served this purpose for centuries, guided not by machines but through refined internal observation.


Breath Holds as a Psychological Practice

The physiological outcomes of kumbhak mirror its psychological effects. When the breath stops, awareness sharpens. Survival signals become louder than mental noise. The nervous system listens. In this stillness, habitual thought patterns soften and deeper layers of experience become perceptible.

Eastern traditions have long taught that stillness is not a retreat from life but a way of encountering it more directly. In the pause between inhale and exhale, the mind becomes more transparent, not as an escape but as a moment of coherence.


Clinical Relevance for Depression and Anxiety

These mechanisms hold particular relevance for conditions marked by disrupted interoception, heightened physiological reactivity and reduced tolerance for internal change. Breath holds, applied safely and with sensitivity, can help individuals recognise subtle internal shifts without becoming overwhelmed. This can support emotional regulation, reduce reactivity and strengthen a sense of internal agency.
